What makes a strong multiple choice question

How many options should a multiple choice question have?

Three. Not four, which is what almost every generated draft produces by default and what most people assume is standard. Michael Rodriguez's 2005 meta-analysis in Educational Measurement: Issues and Practice, synthesizing 80 years of research, found that three-option items hold up as well psychometrically as four- or five-option items, and that you can fit more of them into the same testing time, which improves how much of the material the test actually covers.

The mechanism is not mysterious. The fourth distractor is almost always the weakest one, because writing two genuinely plausible wrong answers is hard and writing three is harder. A distractor nobody selects contributes nothing except reading time, and reading time is the resource you are spending when you could be asking another question instead.

Why three wins

The option count finding sounds like a style preference until you look at what happens to the fourth option in practice. A distractor only does work if some students actually choose it. Measurement researchers call one that nobody picks a non functioning distractor, and the usual threshold is simple: fewer than five percent of test takers select it.

Counted that way, the fourth option mostly is not there. Haladyna and Downing, reviewing 477 items across four assessments, found that roughly two thirds of four option items had only one or two functioning distractors, that over 38 percent of all distractors were selected by fewer than five percent of students, and that none of the five option items had four functioning distractors. The share of items carrying three genuinely functioning distractors ranged from 1.1 to 8.4 percent. In a separate 200 item physician exam, no item had four functioning distractors.

This is the mechanism behind the Rodriguez result. Three options are not optimal because shorter tests are somehow better. They are optimal because the fourth and fifth options were never really working, so removing them costs nothing measurable and buys back the time to ask more questions. That last part is where the actual gain sits: more items covering more of the material is a bigger improvement to a test than any single item edit.

It also gives you a concrete way to review a generated draft. Rather than judging each distractor on whether it looks reasonable, ask whether any student who studied would plausibly pick it. If the answer is no, it is a filler, and deleting it makes the item both shorter and more honest about its guess rate. How to check an AI-generated MCQ before you use it

Cover the options and try to answer the question from the stem alone. If you cannot, the stem is underspecified. That one test takes about five seconds per item and catches most weak questions. The guide is worth knowing about because it sorts flaws by what they do to the score rather than by how they look. There are two families. The first adds irrelevant difficulty, making an item hard for reasons that have nothing to do with whether the student knows the material. The second cues the test-wise, letting a student who studied test-taking rather than content pick the answer anyway. Both distort the result, in opposite directions.

Flaws that add irrelevant difficulty

These make an item measure reading speed, patience or tolerance for ambiguity instead of knowledge. A student who understands the topic still gets the item wrong, which is the worst failure mode an assessment has.

Flaws that hand the answer to a test-wise student

These are the expensive ones, because the item still looks fine and quietly stops measuring anything. A student who has never opened the material can pick the answer from the shape of the options.

Two of these deserve extra attention on generated items specifically, and it follows from how the model works rather than from any defect in the material. A model drafting a correct answer has your source text in front of it, so the correct option tends to inherit the source vocabulary and the source precision, which produces word repetition and a correct option that stands out. The distractors are invented, so they come out shorter and vaguer. Reading only the option lengths, top to bottom, is a fast way to spot it.

What the numbers tell you after students sit the test

Two statistics tell you whether an item earned its place. The p-value is the percentage of students who answered correctly. NBME treats items above .95 or below .30 as carrying little information about the group: nearly everyone got it, or nearly nobody did, so the item did not separate anyone.

The second is the item-total correlation, which compares performance on that one item against performance on the whole test. Positive and large is what you want: students who knew the material got it right. Near zero means the item is measuring something unrelated. Negative is the alarm, because it means students who did worse overall were more likely to get that item right, which usually points to a keyed answer that is wrong or a flaw of the kind listed above. If your platform reports these numbers, sort by the correlation and read the bottom five items first.
