What a generated NCLEX style question looks like

This sample is the kind of item the generator writes from a single cardiac medications slide on loop diuretics. The stem puts the student at the bedside, the options are findings a nurse would actually see on a shift, and the rationale explains why each one is or is not reportable.

That is the difference between a recall question ("What class of drug is furosemide?") and a judgment question. Licensure items test what the nurse does with the information, so the generator is pointed at scenarios, priorities and safety rather than definitions.

Rationale

Loop diuretics waste potassium, and 3.1 mEq/L is below the usual 3.5 to 5.0 range, raising the risk of dysrhythmias. Hypotension with orthostatic dizziness suggests volume depletion and a fall risk. Urine output above 30 mL per hour is adequate, and a small daily weight loss is the expected effect of diuresis.

How to get better NCLEX style questions out of any lecture

Upload the content, not the outline

A slide that says "Heart failure: nursing care" gives the AI nothing to test. Slides with the assessment findings, lab values and interventions produce scenario questions. If your deck is sparse, add the handout or chapter with it.
