The short answer: the NHA phlebotomy passing score is a scaled score of 390 on a scale that runs from 200 to 500. That is not a percentage, and it is not a raw count of correct answers. NHA does not publish a conversion between the two, so nobody can honestly tell you how many questions you are allowed to miss on the Certified Phlebotomy Technician exam.
That last sentence puts this page at odds with most of the search results for the same question, which is why it is worth explaining properly.
NHA states in its candidate handbook that scaled scores range from 200 to 500 and represent a conversion of the candidate's raw score to allow for comparison between different forms of the same examination. The stated purpose is consistency: the same level of performance is required to meet the passing standard of 390 regardless of which form of the assessment a candidate happens to sit.
Read that carefully and the implication is unavoidable. Exam forms are not identical. Some contain slightly harder items than others. Scaling is the mechanism that stops a candidate being penalized for drawing a tougher form. If a fixed number of raw correct answers always produced a 390, scaling would have no job to do.
So when a study site tells you that 390 is 70 percent, or that you can miss 30 questions, it is publishing an estimate in the voice of a fact. NHA has not released that figure, and by the design of the scale it is not a single figure to begin with.
There is no published answer. The exam has 100 scored items, your raw performance on those items is converted to the 200 to 500 scale, and 390 or higher passes. The number of scored items you can miss and still land on 390 shifts with the difficulty of your particular form, and NHA does not release the conversion tables.
The practically useful version of the answer is that you should not plan around a margin at all. Candidates who go in aiming to scrape a pass tend to have one soft domain they have quietly decided to accept, and a soft domain is exactly what a 100 item exam spread across five content areas is built to find.
A large share of phlebotomy prep pages, and a fair number of AI-generated summaries, describe the CPT exam as 100 questions with 80 of them scored. NHA's own published CPT test plan says something different: 100 scored items plus 20 pretest items, which is 120 questions in total.
The 80-scored figure belongs to other NHA credentials, not to phlebotomy. It matters because a practice test built to simulate an 80 question scored exam is calibrated to the wrong specification, and because the 20 unscored pretest items are not flagged during the exam. You have no way to identify them and no reason to treat any question as if it does not count.
Honestly: it is broad rather than deep. Individual CPT questions are not conceptually difficult, and most people who complete a phlebotomy program have already encountered the material. The difficulty comes from coverage. Here is how NHA divides the 100 scored items in the current test plan.

Two domains account for 54 of the 100 scored items. If you have limited study time, Routine Blood Collections and Safety and Compliance are where it goes. The domain most people underprepare is Special Collections, and there is a specific reason for that.
NHA rebuilt the CPT test plan on a job analysis completed in 2024. The previous outline came from a 2016 job analysis and had been in place for years, which means a great deal of phlebotomy review material still in circulation is weighted to the old plan. The most significant move was Special Collections, which went from 7 items to 12. That is a 71 percent increase in its share of the exam.
Blood cultures, glucose tolerance testing, blood bank specimens, pediatric and geriatric draws and therapeutic phlebotomy are no longer a footnote worth seven questions. They are roughly one question in eight. Patient Preparation moved the other way, dropping from 23 items to 20, and Routine Blood Collections came down from 30 to 28.
If your review book predates the change, your study time is distributed against a specification the exam no longer uses. This is the strongest argument for building practice questions out of your own current program material rather than working through a purchased bank: your instructors teach to the plan in force. A book printed against the 2016 outline does not. A phlebotomy practice test generated from the chapters your program actually assigned will track the current weighting by default.
NHA allows three attempts at the CPT exam with a minimum 30 day waiting period between each one. After a third failure you have to wait a full year before testing again. Every retake requires you to re-register and pay the full exam price, so attempts are not free and the third one carries real consequences.
The one genuinely useful thing a failed attempt gives you is diagnostic information. Candidates who do not pass can log into their NHA account and view their score along with a breakdown of which content areas need improvement. Use it literally. If the report flags Processing and Special Collections, spend the 30 day window on specimen rejection criteria, pre-analytical errors, transport temperatures and blood culture technique rather than restudying everything evenly. Even coverage is what produced the first result.
Since you cannot aim at a percentage, aim at confidence per domain instead. The measure that predicts a pass is not how many practice questions you get right, it is how many you answer without having to eliminate two options first. Recognition and recall feel similar when you are rereading a chapter and behave completely differently under a clock.
Three things follow from that:
The certificate and wallet card arrive by US mail within about 7 to 10 business days of the exam being scored, and the credential is valid for two years from its issue date. Recertification takes 10 continuing education credits per two year cycle, where two hours of attendance at a qualified program equals one credit. Excess credits do not roll into the next cycle, so there is no point front-loading them.
One small practical note for the job search that follows. Hospital and reference lab HR departments run inbound applications through resume screening software before a human sees them, and those systems match on the exact credential string. Write "Certified Phlebotomy Technician (CPT)" rather than "phlebotomy certified" and put it in the certifications section, not buried in a job description bullet.
