To make an answer sheet for a multiple choice test, put a numbered column of response boxes on its own page, one number per question, with the letter options printed beside each number for students to circle or fill. Keep the answer key as a separate document. That is the whole design, and the reason it works is that grading becomes a matter of laying one page beside another instead of hunting through a test booklet.
Most of the pain in grading paper tests comes from three specific failures: answers scattered through the booklet so you flip pages per student, a key that does not match the form in front of you, and numbering that drifts after a late edit. All three are preventable at layout time, and none of them require special software.
Because grading speed scales with how little you have to move your eyes. If students circle answers inside the questions, you read a full page to extract four answers. If they record everything in one column on the last page, you read a single strip per student and can grade a class of thirty in a few minutes with the key beside you.
It also makes the test reusable. Booklets that stay clean can be collected and used again next period or next year, which matters if you print for several sections. And it removes the temptation to leave grading marks on the questions themselves, which is how a test copy ends up circulating with the answers visible.

Circling a preprinted letter beats writing the letter freehand for one practical reason: handwritten letters are genuinely ambiguous under time pressure. A hurried C and a hurried G, or a D and an O, will cost you a dispute at some point. Printed options remove the judgment call.
Not for a normal class. Bubble sheets pay off when a machine reads them, when you are grading several hundred students, or when you need item level analysis across a large cohort. For a group of twenty five to thirty five, a plain numbered column graded against a key is just as fast and costs nothing. Buying forms and scheduling scanner time to grade a Friday quiz is usually more overhead than the grading itself.
If your school does have a scanner, follow its form spec exactly rather than designing your own, since the machine cares about mark position and paper size in ways that a homemade sheet will not satisfy.
Print the key on the same layout as the student answer sheet, with the correct letters filled in. Laying two identical grids side by side is much faster than reading a list of answers against a grid, because your eye compares positions rather than parsing text. Add the point value per item and the total at the bottom.
Keep the key as a genuinely separate file, not a hidden column or white text in the test document. Hidden content reliably survives PDF conversion, printing, and opening in a different application, and that is the usual route by which a key gets loose before the test is given. The safest workflow is to generate the test and the key together as two separate exports, so they always match and never share a file.
This is the error that costs an entire grading session, and it has three cheap defenses that work together. Label the form in the header of both the test and the answer sheet. Print the two forms on different colored paper so they are distinguishable across a room. And grade one blank copy of each form yourself, against its own key, before you touch the student stack.
That last step takes two minutes and catches every version of the problem: a key that is off by one, a form whose questions were reordered without the key being renumbered, and the plain mix up of picking up the wrong key. If you are producing multiple forms regularly, generating each version separately is safer than scrambling one by hand, since hand scrambling is precisely where the renumbering error is introduced. A single letter page comfortably holds 50 items in two columns of 25, with a name line and a form label at the top. At 100 items you are into four tight columns and the sheet starts to get error prone to grade, so it is usually better to split a long exam into two sheets by section. Leave a visible gap every five items. It gives your eye an anchor and makes it much easier to spot a student who skipped a question and shifted every subsequent answer up by one.
That shifted answer situation is worth a policy decision in advance. A student who skips item 12 and records everything after it one row early will score close to zero despite knowing the material. If you can see clearly that a whole block is shifted, most teachers regrade from the shift point. Decide what you do before it happens so you do it the same way for everyone.
That running tally is the closest paper equivalent of the item analysis an online quiz gives you free, and it costs nothing to collect while you are grading anyway. An item that two thirds of the class missed is usually a flawed item or a topic you did not actually teach, and either way you want to know before you hand the scores back.
